your insurance company

If you are considering seeking reimbursement from your insurance provider, or if you are calling to see if I am in network, please contact them to find out the following information:

Out of Network Questions

  1. Am I able to see someone out of network and get reimbursement?

  2. Do they cover CPT codes 90791, 90834, and 90837 with diagnosis?

  3. What is my out-of-network deductible?

  4. What is my coinsurance rate for out-of-network outpatient mental health services?

  5. Is prior authorization required?

  6. Do I need a referral before seeking mental health services?

  7. Is there a session limit?

 

 

Some insurance companies make it difficult to receive reimbursement for services. I do not guarantee reimbursement for services. I can provide courtesy billing, which means that you pay me my full fee, and the insurance company reimburses you directly. Alternatively, I can give you a receipt for services, and you can file the claim on your own.
